susan-barbaraWe understand that becoming comfortable in a new environment takes time and the process can be very difficult for some people. That is why Woodhaven Center of Care has a staff of Social Workers that serve to assist patients and their family members with their adjustment to our facility. Our staff of social workers care about the needs of you or your loved one and will do everything within their power to ensure that each resident gets the most out of their time with us. Our full-time skilled Social Workers are experienced in providing counseling to individuals and families during this time of adjustment. Our Social Workers are also able to make referrals to mental health services for patients who may be suffering from conditions such as Alzheimers or dementia. Support for residents begins once they are admitted and continues throughout their stay. When the time comes for a resident to be discharged, our social work staff will coordinate everything they need in order to leave our facility and return back home. Our comprehensive knowledge of community resources enables us to provide discharge planning that suits the needs of each individual and connects patients and families with home care throughout Long Island.

Discharge Planning

Discharge planning begins on admission and continues throughout the patients stay. The goal is to foster the appropriate utilization of healthcare services to ensure a timely and smooth transition to the most appropriate setting for post-rehabilitative care.

The discharge plan assesses the needs of the patient and matches those needs with the appropriate and available options. Medical services, supplies, equipment, education, monitoring, and transportation are arranged as needed upon discharge.

At the time of discharge, both the patient and the primary medical doctor receive a copy of the discharge summary. A follow up appointment with the primary medical doctor is recommended. Prescriptions are provided and reviewed with the patient and/or family, and a copy of the medications is provided when following up with their primary medical doctor.
